Attention members of the public and licensees of the Board:
Changes to the Arizona Administrative Code for the Board of Optometry were approved by GRRC at the January 5, 2016 Council meeting. The amended rules are effective April 19, 2016.
Renewal fee increase is effective April 19, 2016 and is payable as such even if you received your renewal notice prior to that date with the $400 or $420 fee on it.

Current Licensees holding a DEA number only!! If you do not have a DEA number, you do not need to register for the program at this time.
For licensees holding a current DEA number, participation in the CSPMP is mandatory pursuant to A.R.S. 36-2702.
