It is the responsibility of the facility contact to safeguard the user ID and password assigned and provided to their facility. OSHPD is not responsible for loss, misuse, or unlawful activity resulting from the use of the user ID and password by persons to whom facility contacts have delegated reporting responsibility or manipulation of the user ID or password by the facility contact or persons to whom facility contacts have delegated reporting responsibility for purposes other than SB 499 reporting activities. The facility contact is responsible for providing valid data in this report.
