If any administrator meets the continuing education requirement but does not submit his/her NOTARIZED renewal application, renewal fee, a copy of the facility’s state license where you are employed, prior to the expiration date of the current license, a late renewal fee of $275.00 must be paid. IF you renew online, you MUST mail page 2 of the renewal application on or before your expiration date.
