Below are some forms required by the Virginia Marine Resources Commission for various activities. Most are in Adobe Portable Document Format (.pdf). You will need to have Adobe Acrobat Reader installed to view and print forms in this format. To get Adobe Acrobat Reader click on the Get Acrobat Reader icon to the left and follow the directions to download the program. Where possible we have also provided forms in MS Word (.doc) or WordPerfect (.wpd) format. They can be opened, filled out, saved, and printed with one of those word processing programs.

Cobia Reporting Form		1 pg	4VAC20-510-10	Cobia harvest reporting for 2016 is voluntary, but please use the form links to the right if you wish to report your cobia harvest. For recreational anglers, online reporting via the SWJ is encouraged (second link). Charter boat captains should use CBOR for online reporting (first link). The paper forms can be used by both charter and recreational fishermen. Permitting and reporting for 2017 is expected to be mandatory.
