FILL THEM OUT BEFORE YOU ARRIVE.
NOTE:
This workshop is only to obtain or modify current child support. It is not to determine child support arrears or for any spousal support issues. Those issues will not be addressed in the workshop.
OTHER REQUIRED ITEMS
IF YOU ARE EMPLOYED:
Bring copies of your pay stubs
for the past two months.
Bring a copy of your W-2 form
for the prior year.
IF YOU ARE SELF-EMPLOYED:
Bring written proof of your income
for the year to date.
Include a profit and loss statement
for the year to date.
IF YOU ARE DISABLED AND RECEIVING
DISABILITY BENEFITS:
Bring written proof of the
amount that you receive each month.
Bring written verification from
your doctor that you are disabled.
IF YOU ARE UNEMPLOYED AND RECEIVING
UNEMPLOYMENT BENEFITS:
Bring written proof of those circumstances.
IF YOU ARE IN A REHABILITATION PROGRAM:
Bring written proof of those circumstances.
IF YOU WERE JUST RELEASED FROM
INCARCERATION:
Bring written proof of those circumstances.
ALL TAX RETURNS FILED BY YOU WITHIN THE
PAST TWO YEARS.
