Request Information

Agency: Select the Agency that this request is for.

Type of Request:

• Deprovision - Customers request servers be placed in the deprovision state while evaluating usability and the need to decommission. Servers in the deprovision state are suspended from monitoring tools, backup schedules, and powered off. Servers can be powered on, returned to backup schedules, and monitoring tools without major impacts via submissions of Incident Tickets. Servers billing status remain in place until server decommission requests are submitted and processed.
• Decommission - The server decommission is requested once the customer has determined the server functionality and data are no longer required. The server decommission includes wiping-out data on disks; removing the server from the environment. The decommission process is time-consuming process; therefore, it is best to avoid returning a server to an operational state. After the process is started a Work Request will be required. The server billing ends during the decommission process.

Work Request #: - Defaults to N/A. Enter the Work Request Number if applicable. Used by NG Project Managers.

“First Name” through “E-mail” fields - Input contact information of the person submitting this request.

Server Details

Single or Multiple Servers

• Single - Select to Deprovision or Decommission one server. Complete the first four fields and proceed to Section 4 if the type of server you select does not require additional site information.
• Multiple - Select to Deprovision or Decommission two or more servers. Complete the first four fields for the first server, if the type of server you select does not require additional site information, then select the:
  • Add Server button - The first server's information will appear in the uneditable/read-only textbox below. The textbox will populate with subsequent server information. Repeat as necessary.
• Type of Server - Select if the server is a Physical Server at an Agency Location, a Physical Server at CESC/SWESC, or a Virtual Server. If it is a Physical Server at an Agency Location, additional site information will be required.

Server Name - Enter the server name, server network name, or computer name.

Asset Tag - Number and/or letters on the Asset Tag attached to the server or shown on the monthly invoice.

Vital Business Function (VBF)

• Yes - The server contains applications, databases, etc. that are vital to the agency.
  • Disposition - Select the appropriate disposition of the VBF from the drop-down menu. Proceed to Section 4.
• No - The server does not contain any vital applications, databases, etc. Proceed to Section 4.

Special Instructions - Use Special Instructions to note if plans exist to immediately repurpose the hardware, the name, or IP address.

Site Address - Address of the physical server location.

Site POC - The person to contact in case additional information about the server or site is needed to coordinate pickup.

Site POC email - Email address of the Site POC.

Site POC Phone - Phone number of the Site POC.

Hours - Site hours of availability.

Location within Facility - The physical location of the server within the site.

Special Handling - List any special handling requirements, such as protected flooring.

Special Security - List any special security requirements.

Loading Dock Available - Select whether or not there is a loading dock available at the site.

Section 4

Approver Information

Approver's Emails: Input up to 2 email addresses for your Agency's Authorized Account Request Approvers (AITRs, ISOs, and Designees). When this form is submitted, those individuals listed within these fields will receive an email from you via the COV Account Request Website to approve or deny the request.
