IMPORTANT UPDATES!

Thank you for participating in the 2016 Health Impact Program.  HIP closed on Monday, October 31, 2016.  All earned points will be verified and will show up on your account. If you have not already reached  500 points you must claim the reward by Thursday, November 10 at 11:59pm CST to be eligible for the 2016 incentive.  Logon to your Mayo Clinic Healthy Living account at www.bewellstaywell.az.gov.

If you are still missing points for preventive services and activities you have completed, please complete the Preventive Services Form located on benefitoptions.az.gov/wellness and email it to [email protected].  You will also be asked to provide proof of your services.

HIP incentive payout will be sent out in December.
