Narcotics Enforcement Division Controlled Substances Registration Renewal

This application is designed to allow the renewal of controlled substance registration issued by the State of Hawaii.

Please enter your NED registration number and the last 4 digits of the DEA registration number below. Both information are required in order to start the renewal process.

A separate registration shall be required at each principal place of business or professional practice where the applicant prescribes, administers or dispenses controlled substances. (Please refer to HRS 329-32-3e for more information)
