Practice - TERCAP©The National Council of State Boards of Nursing (NCSBN) initiated TERCAP© (Taxonomy of Error, Root Cause Analysis and Practice Responsibility), a national nursing adverse event database to be used by boards of nursing to identify root causes of nursing practice breakdown. The TERCAP© Project meets the recommendation made in the Institute of Medicine's (IOM) Report Keeping Patients Safe concerning the role of Boards in promoting patient safety. The goal of this project is to learn from incidences of nursing practice breakdown in order to evaluate the multiple factors that may be involved in nursing practice errors. Practice breakdown is defined broadly as the disruption or absence of any aspects of good nursing practice. Texas TERCAP© Pilot Project DescriptionIn 2011, the 82nd Texas Legislature passed SB 193 allowing the Board to adopt an error classification system, such as the TERCAP©, for utilization by nursing peer review committees. Consistent with SB 193, the Board is utilizing TERCAP© as a model for identifying practice issues that are normally reviewed by a nursing peer review committee during the peer review process. Once the committee determines that the nursing practice error is not required to be reported to the Board as a part of a complaint or disciplinary process, the incident will be entered into the Texas TERCAP© Online State-Wide Error Reporting System. Information is de-identified and confidential. Recognizing and highlighting factors involved in nursing practice breakdown will promote a better understanding of the etiology of nursing practice errors. Further, evaluating causative factors and developing methods to mitigate nursing practice errors should facilitate a proactive approach to promoting patient safety: an approach that the Board believes is the best way to fulfill its mission to protect the public. The Texas Board of Nursing launched TERCAP© Pilot Project in August 2012 and will continue through August 2016. Pursuant to Section 303.012 of the Texas Occupations Code, the Texas Board of Nursing (Board) has adopted a standardized nursing error classification system for utilization by peer review committees. The Board invited hospital based nursing peer review committees to participate in the TERCAP© Pilot Project wherein de-identified nursing peer review information will be shared utilizing an online error classification system. This sharing of data will permit the development of a pool to analyze nursing practice errors. Error events that are submitted under the pilot are only those peer reviewed incidents that are not required to be reported to the Board as a part of the complaint or disciplinary process.
