Chronic diseases are those conditions which can be managed yet not cured.  Many of these manageable diseases are the leading causes of death in Oklahoma.

The top 10 leading causes of death in Oklahoma are:*

  • Heart Disease,
  • Cancer,
  • Chronic Lower Respiratory Disease (Asthma, COPD, Emphysema and Chronic Bronchitis),
  • Unintentional Injury,
  • Stroke,
  • Diabetes,
  • Alzheimer's,
  • Influenza/Pneumonia,
  • Suicides,
  • Kidney Disease.

When reviewing state rankings, Oklahoma has:*

  • The highest rate of death due to chronic lower respiratory disease in the nation;
  • The third highest rate of death due to heart disease in the nation;
  • The fourth highest rate of death due to diabetes in the nation;
  • The fourth highest rate of death due to stroke in the nation; and
  • The 12th highest rate of death due to cancer in the nation.

These chronic diseases make up the core of the Chronic Disease Service.
Within each disease core there are four areas of concentration.

The areas of concentration include:

  • Education,
  • Screening,
  • Self-Management, and
  • Communication.

The basis for efforts to impact these diseases is data.  In support of statewide efforts, the Chronic Disease Service completed a compilation of documents, which are available for public use.

To view and/or save an electronic version of these documents, or contact the Chronic Disease Service for hard copies, while supplies last.

These efforts could not be achieved without the coordination of both internal and external partners. Some of these partners include:
