How Do I Request a Driving History Report (MVR)?

A Non-Certified copy of your Driving History (MVR) can be downloaded by using our Internet Services.

To request a viewable or certified Driving History Report (MVR) you will need to establish a secure account with DDS. Once you set up your account you may log in using your login and password. You have the following options:

• Viewable version of your Driving History (MVR).
  • This will be available for viewing for 30 days after the date requested.
  • This version is not certified and may not be accepted by your employer, insurance agency or for other parties that would like a certified copy.
  • The fee is $6.00 for a 3 year history, and $8.00 for a 7 year history.

• Certified version of your Driving History (MVR).
  • This version is not viewable and will be printed, certified and mailed to the address that you request.
  • This version will reflect your Georgia Driving History on the date it is printed.
  • A certified copy is usually requested for employment purposes, insurance agencies and others that request a version stamped and certified by DDS.
  • This report can be sent to a 3rd party on behalf of the customer.
  • The fee is $6.00 for a 3 year history, and $8.00 for a 7 year history.

• Payments via Internet Services require a valid credit card. We accept Visa, Mastercard, American Express, and Discover.
• Payments can also be made by mail. A payment coupon will be available for print at the end of your transaction. Mail in payments include: Visa, Mastercard, American Express, Discover, check and cashier check or money order.

In person

A Georgia Driving History Report (MVR) can be purchased in person at any Customer Service Center. Proper identification will be required. If obtaining a motor vehicle record other than your own, the following is required:

• A signed authorization from the licensee naming the person authorized to receive the record. Form DDS-18 (Request for Motor Vehicle Record [MVR]) is available for this purpose, but is not required.
• Authorization form or letter must be original. Faxes or copies are not acceptable.
• The full name, date of birth and Georgia driver's license number of the licensee.
• The person receiving the report for another individual must show proper identification.
• The cost is as follows: 3 year record is $6.00; 7 year record is $8.00.
  • Visa and Mastercard are accepted at all CSC’s. Some CSC’s also accept American Express.

Via Mail

You can obtain a copy of your Driving History (MVR) by submitting a letter (or Form DDS-18) requesting either a 3-year or 7-year MVR. The request should include the following:

• Your full name as it appears on your driver's license
• Driver's license number
• Date of birth
• Complete mailing address
• Self addressed, stamped envelope

To obtain a copy of a Driving History (MVR) for someone else, please complete Form DDS-18  with all required signatures and information.

The fee is $6.00 for a 3 year history, and $8.00 for a 7 year history. You may pay by personal or business check, money order, cashier's check, or credit card. We accept Visa, Mastercard, Discover, and American Express by mail.
