Federal law requires every CDL or CLP holder to “self-certify” as to driving type.

To satisfy the requirements of 49 CFR 383.71, you must check the self-certification category that applies to you. Please check only one category. You must also provide a medical certificate. Please fax a copy of your current medical certificate card to 860-263-5568.

I will operate a commercial motor vehicle both in and outside the boundaries of Connecticut.

 Intrastate

I will operate a commercial motor vehicle only within the boundaries of Connecticut.  

Interstate Transportation

I will operate a commercial motor vehicle both in and outside the boundaries of Connecticut. 

Intrastate Transportation

I will operate a commercial motor vehicle within the boundaries of Connecticut.

You must obtain a duplicate of your current CLP or CDL reflecting any changes based on your self-certification category selection.
