Oklahoma Digital Prairie: Documents, Images and Information

Oklahoma state agencies issue a variety of forms for use by Oklahoma citizens and businesses. The Oklahoma State Government Forms One-Stop Initiative requires the State of Oklahoma Chief Information Officer (CIO) to develop and maintain an online repository to make forms available to the public. State agencies are required to submit all forms in a searchable electronic version for inclusion in this collection.

Since the collection may include older as well as new versions of forms, visitors are urged to pay special attention to the revision or issue date of a form to ensure the appropriate form is being used. Please remember that not all forms contain revision dates.

Background

In May 2011, Oklahoma Governor Mary Fallin signed into law the Transparency, Accountability and Innovation in Oklahoma State Government 2.0 Act of 2011 (HB 1086) into law. This measure contained numerous initiatives aimed at providing greater access to information for citizens, including the Oklahoma State Government Open Documents Initiative and the Oklahoma State Government Forms One-Stop Initiative.

The State of Oklahoma government forms as prescribed in this statute – Forms.OK.Gov – are being made available through a partnership between the Office of Management and Enterprise Services – Information Services Division and the Oklahoma Department of Libraries Publications Clearinghouse.
