Applications Open for Advanced Medical Home Program
The SIM Program Management Office is currently accepting applications for participation in our Advanced Medical Home (AMH) Program. The program is open to 150 eligible primary care practices within Advanced Networks. Selected participants will receive technical assistance support to obtain the Advanced Medical Home designation, which includes NCQA PCMH 2017 recognition.

The SIM Program Management Office, Consumer Advisory Board, and North Central Regional Mental Health Board are hosting a healthcare chat via Twitter at 1 p.m. on Monday, November 21. A tweet chat is an online discussion with other people, using a hashtag (ex: #hcsmct #cthealthchat) to find others discussing the same topic at the same time. The chat is open to everyone, especially Latinos and Latinas and will focus on open enrollment, consumer feedback, and outreach. Click here for more information on how to participate.

Healthcare Innovation Steering Committee approves Quality Council Report
On Thursday, November 10th, the Healthcare Innovation Steering Committee approved the final draft of the Report of the Quality Council on a Multi-Payer Quality Measure Set for Improving Connecticut's Healthcare Quality. The report contains the recommendations of the Quality Council regarding a core set of quality measures for use in value-based payment arrangements. Click here to read the full report.

Value Based Insurance Design Employer Manual for Fully-Insured Employers
The SIM Program Management Office and the Office of the State Comptroller released the Value-Based Insurance Design Employer Manual for Fully-Insured Employers. The manual contains templates and supporting documentation for fully-insured employers interested in developing value based insurance design plans.
