From time to time, workers, players or Members-In-Charge have concerns or questions about how a game is being conducted. If a situation occurs that you are not comfortable with or feel may violate a law, complete the following form to register your concern.

You may also call the office to speak directly to us or print out the Incident Forms (OCG340)
located under Forms/Organization/Reports and mail it in.
