Program Description

VA offers a number of services that can provide you with the support that's right for you. Whether you and the Veteran you care for could use some help at home or you just need someone to listen, we're here to support you.

Your local Caregiver Support Coordinator is a licensed professional who can support you by matching you with services for which you are eligible, and providing you with valuable information about resources that can help you stay smart, strong and organized as you care for the Veteran you love.

Services

  • Extended Care Services: Adult Day Health Care, Home-based Primary Care, Homemaker and Home Health Aide Services, Respite Care, and Hospice and Palliative Care. (Co-pays may apply.)
  • Services for Caregivers: Local Caregiver Support Coordinator, Caregiver Support line (1-855-260-3274), and Support Groups.
  • Services for Caregivers of Post-9/11 Veterans: Stipend, Health Insurance, Counseling, Training.

Eligibility

  • For Extended Care Services and Caregiver Support: VA enrollment, clinical need.
  • For Caregivers of Post-9/11 Veterans: Veteran seriously injured in the line of duty, in need of person care services.

How To Apply

  • The Caregiver Support Coordinator can help with application for VA health care enrollment, extended care services, and caregiver support services.
  • For Caregivers of Post-9/11 Veterans, the application for services is Form 10-10CG.
  • For Extended Care services, the Veteran may need to fill out Form 10-10EC, depending on service connection, diagnosis, and income.
