Finger Print Search

Please supply your 6 digit provider ID along with the individual’s first and last name, birth date, and last four digits of Social Security Number. Click the search button to determine if DCFS already has this individual’s finger print on file. The results are displayed in a popup window (turn off popup blocking for this site). Please print the popup window using the Brower's File and Print menu item and keep a copy in your records for verification.

If the individual’s finger print is NOT on file then proceed with finger print process. A copy of the printed results form must accompany the individual to the finger print facility. If the individual’s finger print is on file then DO NOT send individual for another print.
