ALERT IIS Organization Codes typically follow the form of a two-letter prefix (usually "AL") followed by four numbers. VFC PINs are 6 digits long, either all numbers for public sites, five numbers followed by a letter for satellites, or a single letter (usually "P") followed by 5 numbers for private sites.

Most organizations which participate in the ALERT IIS Program have an individual on staff recognized as the ALERT IIS "Authorized Representative" or the "ALERT IIS Technical Contact." VFC Participating clinics, likewise, usually have a "Primary VFC/AFIX" coordinator on staff, and a staff member responsible for receipt of VFC vaccine deliveries. These individuals should know the clinic's ALERT IIS Organization Code and/or VFC PIN.

Personal immunization records from ALERT IIS can be obtained from your, or your child’s, medical provider. Many schools and daycare centers also have access and can provide you with a copy of your child’s record. The Oregon Immunization Program works to reduce the incidence of vaccine-preventable disease in Oregon. Our staff members seek to identify and promote evidence-based public health best practices to both the public and to health care professionals throughout the state.

In addition to education, the Oregon Immunization Program provides technical assistance and quality improvement for both public and private practices.
