The Oregon Immunization Program works to reduce the incidence of vaccine-preventable disease in Oregon. Our staff members seek to identify and promote evidence-based public health best practices to both the public and to health care professionals throughout the state.
In addition to education, the Oregon Immunization Program provides technical assistance and quality improvement for both public and private practices.
The Oregon Immunization Program is part of the Center for Public Health Practice.
This portion of the Immunization Program website is intended for use by authorized staff members of clinics that participate in, or wish to participate in the ALERT IIS and/or VFC Program.
If you have questions or seek additional information, or wish to obtain immunization records for yourself or your minor child, please contact the Help Desk at (800) 980-9431 or alertiis@state.or.us.
