We offer free resources to answer your questions about lung health, lung disease, and lung cancer and support groups for people struggling with COPD, lung cancer, and other lung diseases.

We offer a variety of classes for chronic disease survivors. The types of classes offered include physical movement-based awareness programs, creative workshops, cancer fitness fundamentals programs, cycling classes for women cancer survivors, and LIVESTRONG at the YMCA.

We also offer the Diabetes Prevention Program, an evidence-based lifestyle change program to prevent type 2 diabetes. A year-long program that helps participants make real lifestyle changes such as eating healthier, including physical activity into their daily lives, and improving problem-solving and coping skills.

Meals for homebound elderly clients.

We provide monthly blood pressure and foot clinics, weekly laughter yoga classes, and exercise class twice a week. We also hold yearly medication management clinics and immunization clinics.

Experience the magic of what happens when people come together to create emotionally empowering experiences for Idaho children diagnosed with cancer and their families. Camp Rainbow Gold is an oncology camp, a sibling camp and a family camp. It’s a hockey night, a ski day and a college scholarship program. Camp Rainbow Gold is a celebration of life. We provide programs and support to the children of Idaho diagnosed with cancer and their family support systems.

Geriatric Care Management with a preventative proactive approach that prefers to help prevent the preventable. We provide advocacy for seniors in the medical system and during hospitalization, rehab and in the facility care system as well.

Dental hygiene services only, no restorative services

We are a hospital-based diabetes and nutrition clinic. We are accredited with the American Diabetes Association. We also see patients for various disorders and diseases for medical nutrition therapy. In addition to medical nutrition therapy, we provide weight loss education and sports nutrition (for those who are recreational athletes or school athletes). We have a fantastic evidence-based weight loss self-management program titled Weigh to Health that is also provided.

Castle Rock Services provides Care Management and Guardianship services to persons with aging related issues, mental health, traumatic brain injury, or developmental disabilities. We also assist family members who have been appointed as a guardian.

The Clearwater Valley Hospital and Clinics Diabetes Self-Management Education/Training/Support Program is for patients who have been diagnosed with diabetes and who have a referral for the program from their primary medical care provider (MD, NP, PA). The program provides education, training, and support in diabetes care to include nutrition, activity, medications, monitoring, risk reduction, problem solving, and coping skills. All goals are tailored to the patient's level of understanding and personal preferences. This is a Medicare covered benefit and most commercial insurances provide coverage as well. Clearwater Valley Hospital and Clinics and St. Mary's Hospital and Clinics offers the National Diabetes Prevention program for anyone 18 and older. This is a 12 month lifestyle change program to reduce the risk of chronic disease and enhance quality of life. The program is facilitated by trained Lifestyle Coaches and is FREE to all participants. Goals include a 5-7% weight loss and an increase of physical activity to 150 minutes a week or more. The program is offered in various sites throughtout all of Clearwater, Idaho, and Lewis counties. Kaniksu Health Services is a federally qualified community health center whose mission is to provide high quality comprehensive and affordable medical, pediatric, dental, behavioral health and veteran care.

It is the mission of the Ketchum Parks and Recreation Department to provide a safe, healthy, and accessible parks and natural resources system, and to provide exceptional youth and adult recreation, arts, and cultural opportunities for the citizens of Ketchum and visitors to our community.

Senior Center is open 3 days a week for activities here at the center and we offer Metro meals-on-wheels. We have a bus that runs four to five times week. M/W/F is around Kuna and Tuesday is to Boise for shopping and medical appointments. On some Thursdays, we have field trips to other centers or attractions around the valley.

As a Master Trainer Select for the National Diabetes Prevention Program, and in conjunction with the Idaho Department of Health and Welfare, Lifestyle Coach Training for the National Diabetes Prevention Program is offered to groups of six or more individuals. The Living Well in Idaho Program is an evidence-based workshop to help people with chronic disease and their caregivers to feel and live better. Qualis Health is working with local stakeholders to provide Diabetes Self-Management Program (DSMP) classes to people with prediabetes and diabetes, particularly underserved populations. If you struggle with managing your diabetes, here is a workshop that can help! The Diabetes Self-Management Program (DSMP) workshops are free and open to individuals living with diabetes or pre-diabetes. The DSMP program was developed by Standford University and consists of six workshop sessions of 2.5 hours each. Each workshop focuses on problem-solving, decision making, and other self-management skills which help participants face the challenges of living with diabetes. Participants are encouraged to attend with a spouse, companion or caregiver. Registration is required. Participants who take the workshops learn about: monitoring blood sugar, preventing low blood sugar, nutrition and healthy eating plan, preventing and delaying complications, dealing with depression and stress as well as foot care.

Madison Memorial Hospital provides an outpatient diabetes education program focusing on teaching people with diabetes how to manage their health and make healthy day to day choices.
The Diabetes Education Dept. at Madison Memorial Hospital has an ADA Recognized Diabetes Self-Managment Education Program, a Weight Management Program that is Medicaid Approved as well as open to the General Public, and has an Insulin Resistance Education Class as well. We are happy to hold classes for the general public as well as individual teaching at the hospital in both Spanish and English. We have an ADA recognized Diabetes Self-Management Program. We also have a Registered Dietician who is the hospital Out-Patient Dietician. She is able to counsel clients with most any dietary problem. We also have a Weight Management program that is open to all ages. Medicare and Medicaid cover the cost of the Weight Management program if the client's BMI is 30 or over or 18 and under.

Madison Senior Citizens Center provides information and helps to refer clients to other community resources. For chronic health issues, we can refer community members to the Stanford University sponsored Chronic Disease Self-Management Program classes. We provide meals for senior citizens (and community members) both at our center and home delivery. We offer social activities at our center Monday through Friday from 11am to 1:30pm. We offer a Fit and Fall Proof Exercise Class twice a week. SHIBA Medicare counseling is also offered.

Basic group nutrition classes and one-on-one nutrition counseling. Classes are free with membership. Medicaid weight-loss program.

We offer healthy congregate meals and meals on wheels. We also offer a variety of exercise classes: Tai Chi, Yoga, and Fit and Fall Proof. We also invite other wellness speakers for our dinners and have an annual health fair.

Free monthly diabetes education classes by Doctor of Pharmacy.

We provide evidence based programs in treating behavioral health such as; DBT, ACT, CBT, Family Functional Therapy, TMS, Medication Management, Boston University Psycho-Social-Rehabiltation Model, Matrix Model, MRT, EMDR and other programs. The tobacco use program uses the following programs; Freedom from Smoking and Not on Tobacco.

The Nampa Recreation Center is a 140,000 square foot recreation facility owned and operated by the City of Nampa. We offer swimming pools, basketball, full weight room, cardio equipment, fitness classes and so much more!

The National Diabetes Prevention Program is an evidence based program incorporating lifestyle changes shown to decrease the risk of developing diabetes in those at risk. The program has also been shown to reduce the risk of complications related to diabetes for those already diagnosed with the disease.

We are a fully integrated clinic offering family medicine, drug and alcohol treatment and mental health treatment.

Nutrition- Idaho North Central District Health participates in the Women Infant Children (WIC) Program. WIC is a nutrition education program that provides growth and health assessments, referrals to community resources and breastfeeding support. WIC also provides nutritious foods that include fresh fruit, vegetables and whole grains that build positive eating habits which can be established and reinforced from the start. There is no cost to families who meet the eligibility guidelines. Pregnant women and families with children up to age 5 are eligible if they live in Idaho and meet the income guidelines.

Oral Health- Sealants are offered to youth ages 5-18 through our school based sealant program. Families on the WIC Program are eligible for sliding scale fluoride varnish, oral exam and nutrition counseling twice per year up to age five.

Physical Activity- Idaho North Central District Health works with volunteers throughout the five county area to offer Fit and Fall Proof exercise classes for older adults. The program is designed to focus on various components associated with balance, including posture awareness and low impact exercised to improve strength, flexibility and endurance. Classes are led by certified volunteers who have received instruction on exercise techniques and personal safety. Components of the Fit and Fall Proof program can reduce the risk of falls, help to maintain static and dynamic balance, take the fear out of falling and build confidence. We also work with childcare providers, work sites, schools and cities to promote healthy active lifestyles through the Idaho Physical Activity and Nutrition (IPAN) Program.

Prediabetes- At Idaho North Central District Health, The National Diabetes Prevention Program is offered at no cost. The Diabetes Prevention Program is a yearlong lifestyle change program that helps individuals prevent type 2 diabetes.

Tobacco Cessation- Free group and one-on-one tobacco cessation counseling to Idaho residents is available. We also work with schools, work sites, and healthcare providers to improve tobacco-users access to, and awareness of, tobacco cessation resources.

We offer vision , hearing screening and blood pressure and blood sugar checks.

We provide Meals on Wheels to home bound seniors, we provide congregate meals 5 times per week, and we are a food bank site distributor.

Southern Idaho Diabetes and Nutrition Education is an American Diabetes Association recognized diabetes education program and offers a variety of nutrition related services. It is housed at Shoshone Family Medical Center, but outside referrals are welcome. Shoshone Family Medical Center is a Recognized Patient Centered Medical Home with health care options and behavioral health services.

St. Mary's Hospital & Clinics has an AADE-accredited Diabetes Self-Management Education, Training & Support Program, with outreach via monthly Diabetes Support Groups in Kamiah & Grangeville from September to May. SMHC's RN, CDE provides basic diabetes nutrition education with option to refer to a contract RD for MNT.

SMHC has an RD-facilitated bi-weekly weight management & lifestyle change support group, Healthy Horizons, in Kamiah, from September to May.
