Veterans with complex needs include those with multiple chronic illnesses (e.g., patients with neurological conditions and secondary conditions) or those with one or more conditions who have complicated social situations and/or use multiple healthcare systems (e.g., Medicare and VA). To improve care and outcomes for Veterans with these needs, CINCCH investigators engage in research activities that enhance the provision of patient-centered, evidence-based care, identify new effective models of healthcare delivery, and add value in care for patients with complex chronic care needs.

Improving Patient Centeredness through New Models of Care
To achieve this goal, the COIN will conduct research to: 1) test new ways to deliver patient-centered care; and 2) measure and enhance patient-centeredness of patient-provider communication and decision making.
Goal #2
Improving Care and Care Delivery for Veterans with High Priority Conditions
This area of research will target Veterans with high priority conditions that are highly disabling or highly service-connected such as spinal cord injury/disorder (SCI/D) or traumatic brain injury (TBI), with an emphasis on improving care and care delivery. Areas of focus include: 1) characterizing, preventing and improving management of chronic and secondary conditions associated with these conditions; 2) improving access to and coordination of care; and 3) improving health care practices in prevention and control of healthcare-associated infections that impact Veterans with these conditions.

Innovative Research to Improve Specialty Care Delivery and Optimize Quality, Outcomes and Values for Patients with Complex Chronic Conditions.
Our efforts in specialty care delivery are focused on chronic kidney disease and cancer. Investigators are working with the VHA National Program for Kidney Disease & Dialysis to improve care delivery for patients with chronic and advanced kidney disease. In the area of cancer, our efforts are focused on developing innovative approaches to measure, track and improve the quality of cancer care. We will leverage our experience and expertise in economics methods and evaluation to optimize care for Veterans with complex chronic conditions, with a particular focus on patients with dual healthcare coverage.
