A Specific License is required to receive and use radioactive material, unless otherwise allowed by rule. Some radioactive material or devices containing radioactive material may be possessed by rule under a General License. See R313-19 for "Requirements of General Applicability to Licensing of Radioactive Material," R313-21 "General License," and R313-22 "Specific Licenses." If this does not apply to you, then go to Part 2: Radiation-Emitting Equipment Registration.
Does your facility wish to obtain a new, amend, or renew a Specific Radioactive Material License?
- YES: Get the Application for Radioactive Material License Form. For new or renewal applications you will also need the applicable guidance documents; then go to the next question.
- NO: Go to the next question.
Are you a medical licensee interested in adding a new Radiation Safety Officer, Authorized User, Authorized Medical Physicist, or Authorized Nuclear Pharmacist?
- YES: Get the appropriate Preceptor Attestation for Medical Use Form from the Form DRC-02A Series; then go to the next question.
- NO: Go to the next question.
Do you or your facility wish to register for, renew, or modify a General License for depleted uranium (DU) in an industrial product or device?
- YES: Get the Registration Form for Use of Depleted Uranium Under General License; then go to the next question.
- NO: Go to the next question.
Do you or your facility wish to register for, renew, or modify a General License for a device that contains a radioactive source with a least 10 millicuries (mCi) of Cesium-137, 0.1 mCi of Strontium-90, 1 mCi of Cobalt-60, 0.1 mCi of Radium-226, or 1 mCi of Americum-241, or any other transuranic isotope?
- YES: Get the Registration Form for Radioactive Material in Certain Measuring, Gauging or Controlling Devices Under General License document; then go to the next question.
- NO: Go to the next question.
Do you or your facility wish to apply to use pre-packaged kits for in-vitro testing distributed under the terms of a General License?
- YES: Get the Registration Form for In Vitro Testing with Radioactive Material Under General License document; then go to the next question.
- NO: Go to the next question.
Does your facility wish to terminate an existing Specific or General Radioactive Material License?
- YES: Get the Certificate of Termination and Disposition of Radioactive Material document; then go to the next question.
- NO: Go to the next question.
Does your facility wish to make payment for a new, renewed, amended, or annual licensing fee?
- YES: Phone the Division of Waste Management and Radiation Control at 801-536-4250; then go to Part 2: Radiation-Emitting Equipment Registration.
- NO: Go to Part 2: Radiation-Emitting Equipment Registration.
