Here are common questions Prevailing Wage Intent and Affidavit (PWIA) users have asked. They are organized in the following categories:

To view approved forms, perform the following steps:

  1. Navigate directly to the View Approved Intents and Affidavits page here.
  2. If you know the form ID number for the form you are looking for, you may enter the number at the top of the page in either the Intent or Affidavit ID field and click on the Get Intent or Get Affidavit button.
  3. If you do not know the form ID number, proceed to next step.
  4. From the Value drop-down menu for Form Type, click to select Intents, Affidavits, or Intents and Affidavits.
  5. In one or more of the Value text boxes, enter your search criteria: Company Name, etc.; from the Match by drop-down menu, click to select Exact Match, Begins With or Contains, and then click Search.

To sort the search results, use the Sort By Field drop down arrow. For example, if you want your search results to sort first by Project Name (A-Z), select “Project Name” in the drop down menu and then select the Search button.

Getting Started: Creating Intent & Affidavit Forms Online

To create a new Intent form, perform the following steps:

  1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
  2. Select whether you are the Prime Contractor (hired directly by the awarding agency) or if you are a subcontractor on this project.  Then select the Create Intent button. If you are the Prime Contractor you are responsible for filling out information specific to the project.  If you are a subcontractor, you need to identify the Prime Contractor’s Intent.  As a subcontractor, if you know the Prime’s approved Intent ID number, you may select that number, otherwise you may search for the project information by identifying the Prime Contractor.
  3. The Intent form is organized so that you answer a few questions and then navigate to the next step. Use the Continue button and the Back to Previous Step button to navigate throughout the form.  Please do not use your internet browser's Back button.
  4. On or after the Intent Details screen, you may save a draft to complete later, by selecting the Save without Submitting button.  At any time you may quit without saving by selecting the Cancel button.
  5. When you reach the end of the Intent form, you will have an opportunity to review your information.
  6. The final step is to pay the $40 filing fee with a credit/debit card or an e-check payment.  Currently L&I accepts Visa, Mastercard, American Express or Discovery. The PWIA system will not remember your credit card or bank information. If you need to file multiple forms, you will need to type in your payment information each time you submit a form.

  • Expand/collapse What do I do if the public agency I am looking for is not listed?
    Please double check the information you are entering to locate the public agency and try again. Also, try entering the first word in the public agency's title. For example if you are searching for KING COUNTY EMERGENCY MANAGEMENT DIVISION, first try entering KING. If you are still unable to locate the public agency, please contact the Prevailing Wage Section by sending an email to PW1@Lni.wa.gov, or call the Prevailing Wage Section at 360-902-5335.
  • Expand/collapse Can I use the system if I don't have a contractor's registration number?
    Yes. You can use the Prevailing Wage Intent and Affidavit (PWIA) system without a contractor's registration license number if you perform service type work such as Building Service Maintenance (janitorial work,) Surveying, and Truck Driving; trades that do not require a contractor's registration license. When you register without a contractor's registration license number, the registration is automatically submitted as a non-contractor registration. The non-contractor registration requires manual approval from Prevailing Wage staff. Please contact the Prevailing Wage Section by sending an email to PW1@Lni.wa.gov, or call the Prevailing Wage Section at 360-902-5335 for manual approval.
  • Expand/collapse How do I create a new Affidavit form?
    To create a new Affidavit form, perform the following steps:
    1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
    2. From the Intents & Affidavits Main Menu, locate the Intent for the project you need to file an Affidavit for.  The list of Intents defaults to the order in which an Intent was created, with the most recently created Intent at the top of the list.  You may view the entire list of Intents by selecting the View Intent History link at the bottom of the Intent list.
    3. When you locate the correct Intent, select the Create Affidavit link. The information you provided on your Intent will populate the Affidavit.
    4. The Affidavit form is organized so that you answer a few questions and then navigate to the next step. Use the Continue button and the Back to Previous Step button to navigate throughout the form. You will need to answer additional questions and will have the opportunity to make adjustments to several fields that were populated from your Intent.
    5. On or after the Affidavit Details screen, you may save a draft to complete later, by selecting the Save without Submitting button.  At any time you may quit without saving by selecting the Cancel button.
    6. The final step is to pay the $40 filing fee, if applicable, with a credit/debit card or an e-check payment.  Currently L&I accepts Visa, Mastercard, American Express or Discovery. The PWIA system will not remember your credit card or bank information. If you need to file multiple forms, you will need to type in your payment information each time you submit a form.

  • Expand/collapse How do I generate an Affidavit from an Intent if I did not file the Intent with my current Secure Access Washington (SAW) account?

    To access Intents that were created in an separate Secure Access Washington (SAW) account, a PWIA Administrator will need to link your current account to an older account, or an account created by a co-worker. If you filed the Intent with a paper form, the Affidavit will also require a paper form. To link accounts you may contact the Prevailing Wage Section by sending an email to PW1@Lni.wa.gov, or call the Prevailing Wage Section at 360-902-5335. Please have the following information available when you call, or in the body of your email.

    1. Company name.
    2. Company UBI number.
    3. Your current Log In ID. (This is the username that you use to access SAW.)
    4. The Intent ID number that you need to file an Affidavit from.
  • Expand/collapse How do I look up prevailing wage rates when I'm in the Prevailing Wage Intent and Affidavit (PWIA) system?
    Please note that if you navigate to the Prevailing Wage Rates web pages, you will leave the PWIA system and may need to log back in. To look up prevailing wage rates from inside the PWIA system, perform the following steps:
    1. From the PWIA Main Menu, click Trades & Licensing.
    2. Click on Prevailing Wage Rates.
    3. To view journey level wage rates click on Look Up, Print and/or Download Journey Level Rates.
    4. To view apprentice wage rates click on Look Up, Print and/or Download Apprentice Rates.
  • Expand/collapse If I filed an Intent using a paper form, can I file the corresponding Affidavit online?
    No, when you file an Intent using a paper form, you will need to file the Affidavit with a paper form as well.  You may download and print paper Intent or Affidavit forms here.
  • Expand/collapse How do I correct a mistake on a form that I submitted?
    1. If a form is submitted, but not yet approved it can be returned for correction. Please contact the Prevailing Wage Section by calling 360-902-5335 if your form has not yet been approved and you realize there is a mistake on the form.
    2. Once a form is approved, the department or customer cannot change it. A new form must be submitted with any corrections and another $40 processing fee paid, if applicable. It is highly recommended that forms be saved as drafts, without submitting, and reviewed for correctness before submitting for approval.
  • Expand/collapse Why am I routed back to the User ID and Password prompt when I try to create a form?
    The most common reason is that your bookmark or favorite is no longer usable to log on to the application. A fresh link may be needed to log on. If you are still unable to log on, please contact the Prevailing Wage Section by sending an email to PW1@Lni.wa.gov, or call the Prevailing Wage Section at 360-902-5335.
  • Managing Your Company Information

  • Expand/collapse How do I change incorrect company information?
    Company information is retrieved from the Contractor Registration Information System (CRIS) database. In order to have your contractor registration or electrical licensing information changed, you will need to contact Contractor Registration and/or Electrical Licensing directly. Documentation may need to be submitted to make changes. Company information may include: address, mailing address, contractor registration number, doing business as, and business name.
    • Contact Contractor Registration at 800-423-7233 for changes to company information related to contractor registration.
    • Send an Electrical Licensing Request for Change of Address form (F500-044-000) for changes to company information related to electrical licensing information.
    • If your company is not required to carry a contractor registration license, contact the Prevailing Wage section at PW1@Lni.wa.gov to make an address change.
  • Expand/collapse How do I change the email address where L&I sends my form approval notification?
    To change the email address where L&I sends your form approval notifications and other notices, do the following:
    • Use the Profile section of the Prevailing Wage Intent and Affidavit (PWIA) system. A link to view and edit your profile is located in the home screen near the Log Off button.
  • Expand/collapse How do I update my PWIA contact information?
    You cannot change your address as you have submitted on your contractor registration license. You must contact the L&I Contractors Program to change contractor registration information. To update your contact information, perform the following steps:
    1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
    2. From the Intents & Affidavits Main Menu, click Profile.
    3. From the User Profile web page, you may edit your phone number and your email address, then select the Save button.
    4. If you need to change the contact name please contact Prevailing Wage at PW1@Lni.wa.gov or 360-902-5335.
    5. To change company information you may need to create a separate Secure Access Washington (SAW) account.
  • Managing Your Online Forms

  • Expand/collapse How do I find a list of forms that have been approved?
    To find a list of approved forms, perform the following steps:
    1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
    2. From the PWIA Main Menu, click View Intent history or View Affidavit history depending on the form type you are looking for.
    3. Use the drop down menu to select Approved forms.

    Please note, if you filed a paper form, it will not display in your online PWIA account. Please see the question: How can the public view approved forms?

  • Expand/collapse How do I find a list of forms that have been submitted but are awaiting approval?
    To find a list of forms that have been submitted but are awaiting approval, perform the following steps:
    1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
    2. From the PWIA Main Menu, click View Intent history or View Affidavit history depending on the form type you are looking for.
    3. Use the drop down menu to select Submitted.
  • Expand/collapse How do I find a form that I have saved but not submitted?
    To find a form that has been saved, perform the following steps:
    1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
    2. From the PWIA Main Menu, click View Intent history or View Affidavit history depending on the form type you are looking for.
    3. Use the drop down menu to select Draft forms, to view forms that have been saved but not submitted.
  • Expand/collapse How can I delete drafts of Intent and Affidavit forms that I created but do not intend to submit?
    To delete drafts of Intent and Affidavit forms that are created but are not intended for submission, perform the following steps:
    1. Log in to your Secure Access Washington (SAW) account and access your Prevailing Wage Intents and Affidavits (PWIA) system service.
    2. From the PWIA Main Menu, click View Intent history or View Affidavit history depending on the form type you are looking for.
    3. Use the drop down menu to select Draft forms.
    4. Click the Delete action for the form you need to delete.
  • Expand/collapse How do I find a form that has been returned for corrections?
    To find a form that has been returned, perform the following steps:
    1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
    2. From the PWIA Main Menu, click View Intent history or View Affidavit history depending on the form type you are looking for.
    3. Use the drop down menu to select Correction, to view forms in correction status.
  • Expand/collapse How do I resubmit forms that have been returned for correction?
    To resubmit a form, perform the following steps:
    1. Log in to the Prevailing Wage Intent and Affidavit (PWIA) system.
    2. From the PWIA Main Menu, locate the form that needs a correction and select the Correction Needed link.
    3. Navigate through the form by selecting the Continue button.
    4. Complete the requested changes, and then navigate to the final screen.
    5. Click Submit for Approval.

  • Expand/collapse How do I report an error?
    To report an error, perform the following steps:
    1. If you are having difficulties logging in to Secure Access Washington (SAW) you may contact the SAW help desk at 1-888-241-7597.
    2. If the error is within the PWIA application, please send an e-mail to PW1@Lni.wa.gov, or call 360-902-5335.
