	How to Apply
	CHIP is now always open for enrollment. If your children don't have health insurance, CHIP may be able to help. Choose one of the following ways to apply:

		• Apply online
		• Download and print application. Mail to: PO Box 143245 SLC, UT 84114-3245
		• Visit your local eligibility office
		• Call the CHIP Hotline for an application: 1-877-KIDS-NOW or 1-877-543-7669
