Evaluation Forms

Evaluators complete an evaluation form for each building they inspect and post. The jurisdiction uses the forms to identify needs for re-evaluation, and to develop dollar estimates of the damage.
Like placards, the forms have been revised based on lessons learned. Primary reasons for the revisions were to:

• Provide local governments with more information for developing dollar estimates of the damage, and


• Provide more historical data on the damaged buildings
