The Alabama Department of Senior Services periodically issues requests for proposals (RFPs) to provide professional services for the department. All ADSS' RFPs are posted on the Alabama State Comptroller’s website as well as this website and will remain posted until the proposal due date listed in the RFP.  ADSS maintains a separate list of potential RFP recipients to mail notice to taken from those sent notice on previously offered RFPs by the Department for the same service and also receives lists from the Alabama State Purchasing Department.  For more information on vendor registration, visit the State of Alabama Purchasing Division at www.purchasing.alabama.gov

REQUIRED: Vendor Disclosure Statement

Section 41-16-82, Code of Alabama 1975 requires the disclosure statement to be completed and filed with all proposals, bids, contracts, or grant proposals to the State of Alabama in excess of $5,000. The disclosure statement is not required for contracts for gas, water, and electric services where no competition exists, or where rates are fixed by law or ordinance. In circumstances where a contract is awarded by competitive bid, the disclosure statement shall be required only from the person receiving the contract and shall be submitted within ten (10) days of the award.

DATED, AND NOTARIZED PRIOR TO SUBMISSION.
