CASE REVIEW COMMITTEE REFERRAL GUIDELINES

Procedures for Referring to the Case Review Committee (CRC):  The Chairperson or Vice-Chairperson of the County Children's Services Facilitation Team (CSFT) should submit the following information to the Multiple Needs Child Office by the third Tuesday of the month to be staffed on the fourth Tuesday of the month:

 All recent (previous 6 months-1 year) material reviewed by the County Children's Services Facilitation Team.

 

bullet

Minutes of the County Children's Services Facilitation Team "staffing" of the case. (i.e., What were the needs identified by the County Children's Services Facilitation Team?)

bullet

 A copy of the “plan” submitted to the Court (if the Court is involved), by the County Children's Services Facilitation Team.

bullet

Completed CRC referral form signed by the Children's Services Facilitation Team Chair

bullet

Submit written case summary of local efforts to address needs. 

Things to consider when referring a case:

Background information.

bullet

Identify strengths and needs, match needs to services and desired plan from County Children's Services Facilitation Team.

bullet

Service objectives and priorities.

bullet

Identify service providers.

bullet

Make County Children's Services Facilitation Team assignments (including case management responsibility) and recommendations regarding funding.

bullet

Update any changes in unresolved cases, include progress on assignments, assuring service delivery, funding, and locating placement/service (if needed). 

                Update Cases:

bullet

Should have an update the first month after service arrangement or placement.

bullet

Written quarterly reports should be submitted by the County CSFT to the MNC Office.

bullet

Should have updates more often when a service arrangement is expected to be of short duration and a new service arrangement or placement alternative will be necessary.

Overview:

The MNC Office will provide the CRC of the SCSFT with the referral packet received from the County CSFT.  The MNC Office will disseminate appropriate documents to the CRC members to be reviewed prior to the staffing on the fourth Tuesday of the month.  Upon completion of the CRC staffing, the MNC Office will submit recommendations to the SCSFT on the second Tuesday of the month for consideration.  The SCSFT will then approve or disapprove recommendations.  Final SCSFT recommendations will be provided to the Chairperson of the CCSFT by the MNC Office.
