Immunizations

Florida Department of Health in Sumter County

Immunizations are provided free for all children, from birth through 18 years. Before any child may enter daycare or attend school, they must be current on their vaccines. Proof of vaccinations are required. Immunizations can protect your child, family, friends and their families and the community.

Information Statements

The following vaccine is available for uninsured adult clients, age 19 years or older at no charge:

  • Human Papillomavirus (HPV4) Vaccine
  • Tetanus-diphtheria-acellular pertussis (Tdap) Vaccine
  • Pneumococcal
  • Polysaccharide (PPSV) Vaccine

Religious Exemptions 

The DH 681 form is issued only by county health departments and only for a child who is not immunized because of his/her family’s religious tenets or practices.  This form must be signed by the parent affirming the written statement that a
religious conflict exists. Exemptions for personal or philosophical reasons are not permitted under Florida law.
