NOTICE OF PRIVACY PRACTICES
A new federal law requires us to give you a Notice of Privacy Practices that the Department of Behavioral Health and Developmental Services must follow in order to protect a patient’s health information.
This Notice informs you of three things:
1. It tells you how we may use and disclose your health information,
or the
health information of the person you represent.
2. It tells you what legal rights you have under this new law.
3. It tells you what our legal duties are under this new law.
