Motor Vehicle Collision Report Form - If you wish to file a report for a collision, please go to the Collision Reports page and follow the form instructions to submit to the Washington State Patrol.

The Uniform Washington State Authorization to Tow/Impound and Inventory Record is maintained at the Office of Printing and Imaging Services (OPIS) at their Fulfillment Center (in Tumwater, Washington). (OPIS was formerly known as the State Printer.)

To order these forms, you must create a "myFULFILLMENT" account with OPIS. Please visit their web site at https://prtonline.myprintdesk.net/DSF/storefront.aspx. From there, click on "Click here to register."

Once the registration process is complete, navigate to the myFULFILLMENT home page and click on the "Fulfillment(By Agency)" link on the left. Navigate to the "State Patrol (WSP)" link, then click on the title of the form.
