Establishment Licenses

Q: What is an establishment license and when is it required?

A: An establishment license is a license that is required where there is more than one licensed massage therapist practicing massage therapy at a single location.

Q: What is an establishment registration and when is it required?

A: All locations where therapists regularly engage in the practice of massage therapy shall register with the board as a massage establishment.  For a solo practitioner there is no fee for this registration unless it is late.

Q: I am a licensed massage therapist. Do I need an establishment license if I rent space in my office or contract with another therapist?

A: Yes. If you are renting space in your office or contract another therapist, then there is more than one massage therapist practicing in that location.

Q: If I am renting space to another licensed massage therapist, and they are not my employee, do I still have to have an establishment license or an establishment registration?

A: Yes. The requirement for an establishment license /registration has nothing to do in regards to employees, contractors, out-calls or space renting. If more than one massage therapist is practicing in a single location then an establishment license must be posted in that location. If only one massage therapist is practicing in a single location, an establishment registration must be posted in that location.

Q: I own a salon and/or spa. Do I need an establishment license or an establishment registration?

A: If you have more than one licensed massage therapist practicing in your salon and/or spa, then the answer is yes - you need an establishment license.  If you are the only practicing massage therapist, you need an establishment registration.

Q: Do I have to be a licensed massage therapist to have an establishment license?

A: No. Licensed massage therapists practice in many settings. They work in their own private practice, salons, spas, physical therapist offices, chiropractic offices, fitness clubs, hospitals and wellness centers among other places. If you are not a licensed massage therapist but you are the owner of a location where more than one licensed massage therapist works, you must have an establishment license.

Q: How much does an establishment license cost?

A: The cost of an establishment license is $100.00 per year.

Q: When does the establishment license or establishment registration expire?

A: The establishment license and establishment registration expires on March 31 of each year.

Q: When is the establishment license and/or establishment registration due for renewal?

A: The establishment license and/or establishment registration renewal must be postmarked on or before March 31 of each year.

Q: How do I renew or apply for an establishment license or establishment registration?

A: All applications and renewal forms can be found on the board's website. Complete an application and mail it to the board office, along with the $100.00 cashier's check or money order renewal fee to receive your establishment license, or apply online and pay with a credit or debit card. This must be done on or before March 31st each year to avoid a late fee.

Q: Where must I display my establishment license or establishment registration?

A: The establishment license or establishment registration must be displayed in plain sight where it is visible to the public.
