Charitable Gaming Forms and Applications
The Missouri Bingo/Pull-Tab License Application should be completed by organizations wishing to conduct bingo on a regular basis or by organizations wishing to conduct bingo during a special event.
The Missouri Pull-Tab Only License Application should be completed by organizations wishing to conduct a game at which only pull-tab cards may be used having a duration not more than 24 hours. Organizations are limited to 15 pull-tab only licenses per calendar year.
Multiple organizations, not to exceed five, may join together to apply and may receive a single license to conduct the game of bingo in accordance with Section 313.010 RSMo. Each organization must meet the requirements for a bingo license on an individual basis. The multiple organization license allows two-year members from each organization to combine their efforts in managing and conducting a bingo event on a regular basis.
