Applications

There are five (5) different applications for licensure. Print the paper application packet that meets your needs and review it in its entirety. After reviewing the appropriate application packet, select the online application link and complete the application for submission to the Board.

1.	Administrator in Training Program (never before licensed): AIT Application packet or Online AIT Application
2.	Endorsement/Reciprocity (licensed in another state): Endorsement Application packet or Online Endorsement Application
3.	Temporary Permit (Employing facility has urgent/immediate need for administrator licensed in another state to begin work at the facility): Temporary Permit Application packet or Online Temporary Permit Application
4.	Reinstatement within 3 – 5 years of lapse: Reinstatement (3-5 yr) Application packet or Online Reinstatement (3-5 yr).
5.	Reinstatement less than 3 years of lapse: Reinstatement (<3 yr) Application packet or Online Reinstatement (<3 yr) Application.

Applicants for licensure must pass the state examination. (Except Reinstatement <3 years). Order a State Study Packet here.
