RA, NG, RESERVE ID CARD RENEWAL/UPDATE

  • Old Common Access Card
  • The following documents as it applies:
    • Enlisted Record Brief (ERB)
    • Officer Record Brief (ORB)
    • Reenlistment Contract/ Extension
    • Promotion Order
    • Oath of Office (DA FM 71)

NOTES: MILITARY PHOTO MUST BE WITHIN MILITARY GROOMING STANDARD per AFI 36-3026

NEW MARRIAGE

  • Marriage Certificate (Original or Certified Copy)
  • Valid State issued Photo ID or Birth Certificate
  • Social Security Card or Letter from SSA with SSN# and Name
  • Final Divorce Decree, if prior marriage

LEGAL CUSTODY/WARDS

  • Final Legal Custody, Guardianship Court Order for at least 12 consecutive months.
  • Birth Certificate (Original or State Certified)
  • Social Security Card or Leter from SSA

DEPENDENTS CHILDREN AGES 21-23

  • School Certification with the following Information:
    • Date Start of School
    • Expected Date of Graduation
    • Must State Full Time or at least 12 Credit Hours
    • POC, Name, Phone Number, Signature of Registrar
    • Must be on official Letterhead from Accredited School

OTHER ELIGIBLE DEPENDENTS

Parents or Parents In-Law

  • Approved Dependency Determination
  • Birth Certificate or State Issued Photo ID
  • Social Security Card

UNREMARRIED FORMER SPOUSE

YOU MUST MEET ALL THE CRITERIA

  • Final Divorce Decree (for initial ID Card only)
  • Old ID Card, Valid State Photo ID or Birth Certificate
  • DD 214 or Statement of Service
  • Death Certivicate (if applicable)

CRITERIA:

Only Spouse with at least 20 years of marriage or more, 20 years or more service creditable eligibility for retired pay, 15 or 20 years or more years of marriage must overlap during the member performed creditable service may receive benefits.

 

If remarries will loses ID Card and entitlement. If a former spouse becomes unmarried through divorce, annulment or death, he/she may be eligible for reinstatement of ID Card for Commissary, Exchange and MWR only.

RETIREE

  • Old ID Card
  • DD FM 214 or Retirement Orders
  • 20 OR 60 Years Letter

NEW CHILD/STEPCHILD

  • Birth Certificates - includes at least one parent’s name.
  • Social Security Card or Letter from SSA with SSN# and Name.
  • Confirmation of Birth signed by any attending physician or medical personnel.

NOTE: Child less than 10 yrs old may not received an ID Card Exemptions are:

  • Single Parent
  • Dual Military Parents
  • Child residing outside the Sponsor Household

CHILD BORN OUT OF WEDLOCK

  • Court Order Establishing Paternity or Approved Dependency Determination or Voluntary acknowledgement of Paternity:

  • NOTE: All documents must be translate in English

MAILOUT PROGRAM

Sponsor resides in another State and no POA is available:

  • Old ID Card
  • Current Address of Sponsor

NOTE: Dependent will be issued 90 day temporary ID

Due to Sponsor of Dependent incapacitation or Immobility:

  • Notarized 5X7 photo of applicant
  • Notarized DD Form 1172

NON-APPROPRIATED FUNDS (NAF)

  • DD FM 1172-2, Signed by Signature Card holder
  • Two forms of State or Federal Issued Valid ID

DISABLED AMERICAN VETERANS

  • VA Letter stating 100% combined rating of disabilities
  • DD FM 214
  • State or Federal Issued Valid Photo ID
