The NASA ASRS Reporting Forms (General, Air Traffic Control, Maintenance, and Cabin) are normally printed by NASA on double-sided, legal-size (8 1/2 x 14) paper, but many users cannot easily print two-sided, legal-size pages. NASA ASRS forms are provided here as Adobe Acrobat single-sided, letter-size (8 1/2 x 11) forms, thus you will have to print two pages for the form. An extra page has been added to permit additional narrative, if desired.

- Fill out the form on your computer, print the completed form, attach all pages together, enclose in an envelope, seal, affix sufficient postage, and mail to ASRS at the address below, or
- Print the uncompleted form, fill it out by hand, attach all pages together, enclose in an envelope, seal, add sufficient postage, and mail to ASRS.
