FINANCIAL DISCLOSURE FORMS

FORM # DESCRIPTION AVAILABLE FORMATS
OGE-278 Senate-confirmed Presidential appointees, all SES employees (and equivalent) and all Schedule C staff are required to file public financial disclosure reports, OGE Form 278s (formerly SF-278s), within 30 days of entering and leaving a position at USDA, as well as annually while serving in a covered position. Annual reports, covering the previous calendar year, are due each year by May 15th, unless you obtain an extension to file. Reports are submitted electronically using the Office of Ethics FDONLINE filing system. If you already have an account please submit your OGE 278 using FDONLINE, via: https://fdonline.ntis.gov/ (If you are new to the system you will need an electronic email invitation to access and setup your account on the FDONLINE system. Please email DAEO.Ethics@oe.usda.gov for account setup/access.

The Stop Trading on Congressional Knowledge (STOCK) Act requires OGE-278 financial disclosure report filers to submit a Periodic Transaction Report (OGE-278-T) to report certain financial transactions. More information on the STOCK Act may be found at http://ethics.usda.gov/rules/stockact.htm. Submit your OGE 278-T using FDONLINE, via: https://fdonline.ntis.gov/ 		Only Electronic Submissions via the Office of Ethics FDONLINE system will be accepted.

OGE 450 and 450A Career staff below the SES level may be required to file confidential financial disclosure reports (OGE Form 450 or OGE Optional Form 450-A) if they make decisions or recommendations that can have a financial impact on the Department's contractors or the commercial interests USDA services or regulates. New Entrant reports are due within 30 days of entering a covered position. Annual reports cover a calendar year and are due each year by February 15th, unless you obtain an extension to file.
