Lifestyle Change Programs

We understand that managing a chronic illness can be time consuming, complex, and difficult. Lifestyle change self-management involves "the tasks that individuals must undertake to live well with one or more chronic conditions.These tasks include having the confidence to deal with medical management, role management, and emotional management of their conditions".

Often times, individuals with chronic illnesses may be asked to monitor symptoms of their condition, adhere to medication regimens, stop smoking, reduce alcohol consumption, modify diet, lose weight and increase exercise, and more. Patients often have difficulty performing these tasks, and this is where lifestyle change programs can provide the support and training needed to become healthier.
