Patient Administration
The Patient Administration Division’s purpose is to maintain patient medical records and assist patients in retrieving medical information pertinent to their ongoing medical care. If you are a Soldier or Family member who is new to the area, we strongly encourage you to stop by our outpatient records department and request your medical records from your previous unit. .

Medical Records
The Medical Records Section falls under the PAD and can be located at the main desk found at the entrance of the Dr. Mary Ed Walker Center in the lobby. Medical Records are the property of the U.S. Government and are maintained for the benefit of the patient. The Medical Records Section and Release of Information staff have the responsibility for ensuring all medical records held in their custody are safeguarded from loss or tampering, and that the information in the records is released only to authorized individuals. A valid ID card is required to request a copy of your medical records. Beneficiaries are entitled to one free personal copy of their medical record, per AR 40-66. Additional copies will be assessed an adminis- trative fee and charges per page. Due to the large volume of requests, customers requesting a copy of their medical records should submit their requests in writing no less than 45 days in advance.


WACH Medical Records Section
Medical records for military family members, retirees, and WACH, DENTAC, and MEDDAC Soldiers are maintained at the Medical Records Section at the MEWC. It is recommended that a medical record be established at the WACH or MEWC for every Family member to be seen for medical treatment on Fort Irwin.


Release of information(ROI)
This department handles requests for copies of medical records. One free personal copy per person is author- ized, additional copies have a fee per page (call number below for fee information). To request a personal copy of your medical records, please visit the WACH Medical Records/ROI window to fill out your request.


Requesting Information:
Patients may request copies of their medical records using DD Form 2870 (Authorization for Disclosure of Medical or Dental Information) available online: www.dtic.mil/whs/directives/infomgt/forms/formsprogram.htm by mail or by visiting ROI reception, located in the Patient Administration Room of the MEWC.


Archived Records:
The WACH maintains outpatient records for a period of 2 years after the patient’s last medical encounter (excluding active duty military and their dependents). Any records from 2015 will be at the AMEDD Records Processing Center. Any records from 1993 to 2014 will be at the VA, which will need to be requested on Standard Form 180. Improving Your Patient Experience: APLSS, ICE and the WACH Customer Survey Card.
Suggestions or improvements on how we may better care for you and improve patient services are not only welcome, they are actively invited by the command and staff at the WACH.

In order to get your opinion on whether we are meeting your expectations, we invite you to fill out a WACH Survey Card, found throughout our facilities after you use our services. For those who prefer to use online surveys the Fort Irwin Interactive Customer Evaluation (ICE) is a web-based tool that collects feedback on services provided by various organizations throughout DoD including the WACH. All ICE comments, complimentary or critical regarding the WACH are seen by the hospital Commander with an eye toward improving the patient experience. For this reason, the ICE is the best way for patients to lodge complaints. All survey information is kept anonymous and confidential unless you specifically direct otherwise.

APLSS, the Army Provider Level Satisfaction Survey, is a survey that is administered by the Office of the U.S. Army Surgeon General. The survey focuses on issues ranging from how the provider communicated with the patient during the visit to questions regarding access to care, cleanliness of the facility, and courtesy of the staff. Patients are randomly mailed a letter asking them to complete a brief questionnaire regarding their care at the WACH. The survey consists of 24 questions.

Completing the survey gives you, the patient, the opportunity to have your voice heard by WACH leadership. We use the survey results to identify areas for performance improvement. You also have the ability to formally recognize people or services that performed exceptionally. Additional funds for your hospital are also generated based on your responses and overall satisfaction ratings.

We take these surveys very seriously and use your comments and suggestions in our continual effort to improve our services, procedures an facilities to serve the community better.
