If you haven’t already done so, complete the Sponsorship Program Counseling and Information Sheet (DA Form 5434) and send it to the
RHC-C HQ Program Manager. You may also request information for your school-aged children at this link to JBSA-Fort Sam Houston MWR.

Outbound Soldiers

One of the most important pre-move actions you can take is to request a sponsor. Please complete the Sponsorship Program Counseling and Information Sheet, (DA Form 5434) and see your unit's sponsorship coordinator.

Sponsorship Training
