Juror Qualification Questionnaire

You may have received a questionnaire for the purpose of determining whether you are qualified to serve as a juror in this court pursuant to federal law. The questionnaire is not a summons for jury service.

We encourage you to complete the questionnaire online by clicking on the link at the bottom of this page. In order to log in you will required to enter the following information:

Your unique 9-digit participant number located to the right of your name and address on the questionnaire.
The first three letters of your last name, exactly as shown on the questionnaire. If your name is misspelled on the questionnaire or has changed, you will have an opportunity to correct it after you log in.
Your date of birth.

You will initially be presented with a screen to verify your name, address and contact information. Then click "FINISH" and you will be taken to the main body of the questionnaire. After completing the questionnaire, you will have an opportunity to review your answers. After that, click "CONFIRM". You will then see a confirmation screen. If you do not get the confirmation screen, please contact the San Francisco jury office at the number shown below for assistance.

Some items on the questionnaire will require you to enter information in a text box. These boxes may not be left blank. If appropriate, you should fill in "not applicable" or "n/a".
