DENTI-CAL NPI REGISTRATION INSTRUCTIONS
After you receive your NPI(s) from NPPES, you must register your NPI(s) with Denti-Cal. Providers may register their NPIs through one of two options: 1) Submitting a hardcopy registration form by mail, or 2) Register the NPIs on the web via the Denti-Cal NPI Collection System.
Providers must register an NPI(s) to all existing Denti-Cal Provider Numbers. In most cases, providers will need to register an NPI for their billing provider number and an NPI for their rendering provider number.

Providers may fill out the NPI Registration Form (DHCS 6218) for their billing and/or rendering provider numbers.
Group providers may use the Registration Form for Group Billing Providers to register their rendering providers' NPIs.
Denti-Cal NPI Collection System
When registering your NPI(s) via the Denti-Cal NPI Collection System, you will need to provide the following information:
Billing Providers
- Denti-Cal Billing Provider number, service office number, and PIN.
- NPI assigned by the National Plan and Provider Enumeration System (NPPES).
Rendering Providers
- Denti-Cal Rendering Provider number and last 4 digits of SSN (Social Security Number).
- NPI assigned by the National Plan and Provider Enumeration System (NPPES).
The Denti-Cal NPI Collection System allows Denti-Cal providers to register one NPI for each active Denti-Cal provider number.

Updating Your NPI(s)
Changes or updates to the NPI cannot be made using the Denti-Cal NPI Collection System. A provider changing or updating an NPI must notify the Denti-Cal Telephone Service Center (TSC) at 800-423-0507.
